Simple clerical errors, where a Purple Heart is denoted in military records but was simply omitted from a Report of Separation, are corrected on site at the National Personnel Records Center through issuance of a document known as a DD-215.
Modern day Purple Heart presentations are recorded in both hardcopy and electronic service records.
In such cases, NPRC searches through unit records, military pay records, and records of the Department of Veterans Affairs.

As for the part about the wound not being inflicted by the enemy, in which case the shrapnel's source would have been "friendly fire," that in itself doesn't exclude a Purple Heart.
